ENROLMENT FORM

Setting up a new stroke centre or maintaining the high standards expected of a centre of excellence is a complex task. Together with our expert steering committee we have created a set of tools that are aimed at solving this complexity. The enrolment form lists the pillars of the Angels Initiative and can be used to officially enrol a new hospital, and includes hospital name, address, and an agreement to our terms and conditions. The form is an agreement by hospitals to set up a new stroke-ready hospital or to optimise the stroke treatment in their hospital.

HOSPITAL RESOURCE FORM

Evidence and experience have shown that effective co-ordination of resources and processes is necessary to become a unit that consistently treats stroke patient effectively and safely. This document captures important information about your hospital’s structure regarding stroke management. It was designed to analyse the current situation in your hospital in terms of treatment of stroke, and to compare it with best practice standards. Answers to the various questions should reflect the current state and not the desired future state of stroke management in your hospital. The Angels Initiative provides various tools including protocols, training resources and quality monitoring using the ESO Excellence criteria to assist with the action plan implementation as and where it is needed. All information contained within this form is strictly confidential and will only be used to help create an action plan within the context of the Angels Initiative.

Body Interact Script Files

Download here the Body Interact Script files with detailed scenario information and solutions for all Body Interact Cases we have available for the Angels Initiative project.  The script files include information about patient characteristics and parameters, physical examination and monitoring findings on admission, all possible dialogue lines, diagnostic strategies, baseline scenario behavior, optimal clinical approach, treatment priorities and assessment questions. Stroke Bag assets

Stroke Box Legal Wording

A legal document that describes the purpose of the stroke bag and what exactly is provided by the Angels Initiative team in order to get approval for its distribution. It is important to make clear that this stroke bag will not be given to individual HCP’s, but rather to hospitals as an item of medical utility to improve treatment of stroke. This falls under Article 9 of the EFPIA HCP code.

Stroke Kit Inventory

A well-stocked Stroke Bag is critical to ensuring a seamless patient experience for each stroke patient. Within this kit we have included all the forms you require, including the Induction Documents, Process Forms, Quality Forms, Reference Documents, Wrist Bands and Labels. We have made a recommendation regarding the medical supplies you will require; however, please use equivalent products relevant to the supplies available in your institution. Please ensure the Stroke Bag is restocked after each patient treatment.

Simulation Brochure

One of the most effective tools we use to identify bottlenecks and optimise stroke care is ‘dummy patient simulation training’. Evidence shows that hospitals using this training regularly, manage to significantly improve their performance in the long term. Focusing on the patient pathway, medical teams are supported by Angels consultants to identify bottlenecks in their hospital, and find appropriate solutions to optimise stroke care. This brochure serves as an introduction to simulations for hospitals, with detailed descriptions of each step: Introductory Meeting, 1st simulation, debriefing and discussion, 2nd simulation, debriefing and discussion.

Simulation Checklist

This checklist gives an overview of all steps of a simulation and which actions and materials need to be considered in each phase. Simulation Presentation

A PowerPoint presentation that briefly introduces the Angels Initiative and the importance of simulations. Each step of the simulation is depicted so the hospital team has a clear idea what to expect.

Time and Actions Tracker

A table to be used during a simulation. It lists all required priority actions, and tracks whether they were performed or not, and at which time point after start of the simulation. It also includes fields for Action points and Training for each item, as well as a field to note general objectives and corrective actions discussed during the debriefing.

Simulation Timeline

An excel sheet where the initial time, length and total duration of each action done during the simulation can be tracked. The inserted values are automatically plotted on a bar graph. This provides a comprehensive overview of the timing of each action item, and where life-saving minutes could be saved and the overall pathway improved.

Action Item List

A document that can be completed after debriefing of a simulation, with a graphic depiction of which actions are needed to reduce DTN times. It explains the feasibility of incorporating these actions into daily practice, and what the impact will be once they are performed. Provides a table where it can be noted which person of the stroke team will implement which action and by which date this should be accomplished.

A PPT file on "How to run an effective debriefing". It will give you some important knowledge and key points to keep in mind for your simulation debriefings in hospitals. The presentation can be printed as an A5 booklet and distributed to everyone involved in the debriefing meeting.

Simulation Release form

This legal document gives an overview of the Angels Initiative Simulation Training. The hospital has to sign that they consent to performing a simulation in their hospital and that permission for filming is granted. It is also stated that the recorded video remains property of the hospital unless agreed otherwise.

Patient Consent Form

Legal document for simulated patient. In this consent form, the "dummy patient" agrees to indemnify the hospital, its staff and its associates from any liability that may arise as result of an adverse event that may occur during the course of the dummy runs. They agree to participate as “Simulated Patient” voluntarily and of their own choice.

Photo and Video Release Form

Legal document that may be signed by a hospital representative after the simulation. By signing, they consent that all photos, videos and voice recordings can be used by the Angels Initiative for public relations and promotion purposes, such as website or presentations.

GoPro Tutorial

Simulations are performed using the GoPro Hero 5 and the karma Grip to increase capture quality and stability. This document gives precise instructions on how to install the correct GoPro settings and serves as a tutorial on how to properly use the device.

Simulation Vests

Unisize cotton vests in different colours with the following prints: Nurse (blue), Doctor (purple), Patient (red), Ambulance (yellow), Porter (green). For distributing to the people involved in the in-hospital simulations to clarify their roles.

EMS Draft Memorandum of Understanding

Template used for an efficient and transparent agreement between EMS and hospitals in a region, aimed at the optimisation of the hyper-acute care of patients with suspected stroke. The primary objective of the Memorandum of Cooperation between the two organisations is to minimise delays in appropriate stroke treatment. The document includes: Regional background, focus areas and specific goals, both from the hospital perspective and that of the emergency medical service. It also determines future tasks related to implementation of the project for both organisations – the hospital stroke team and EMS.
