The Welcome Enrolment Pack is designed to support hospitals as they join the Angels Initiative, marking an important milestone in their commitment to improving stroke care. This pack helps create momentum and sets the tone for a successful start to the project.

This Initial Presentation provides a structured and consistent approach for introducing Angels to hospitals. It combines data-driven insights with an emotional appeal to create impactful and persuasive messaging aimed at securing hospital engagement.

The presentation offers a selection of standardized slides to ensure clarity and consistency. While it is not mandatory to use all slides, they serve as guidance for delivering a compelling initial message.

Note that the presentation was build as part of the new lean consultancy project. However, for countries, that are not yet enrolled in the Lean Consultancy Project, they can still use the presentation, excluding slides 50-57 & 59.

Currently only the English version is available. You can translate and adapt for hospitals using relevant Angels website dashboard screenshots for country and global overviews. Please share translated versions with the Angels Core Team so we can add it here in the CRP.

The Hospital Resource Form (HRF) is designed to capture key information about the hospital’s infrastructure, staffing, and stroke care capabilities. This data enables tailored consultancy and identifies opportunities for lean improvement.

At the start of the Discover Phase, the Angels Consultant uses the HRF to document findings across six sections, covering all steps in this phase.
By the end of the Discover Phase, the completed HRF provides a solid foundation for the next stages of the lean consultancy process.

The HRF is currently in a pilot phase. Feel free to add lines in the interview sections if you identify missing questions and use the current version as a pilot tool. Feedback is welcome and will be collected at the end of the piloting phase.

The Waste Walk Poster supports identifying and categorizing waste within the stroke pathway during the Discover Phase.
In Lean Six Sigma, activities are classified as value-adding (VA), necessary non-value-adding (NNVA), or non-value-adding (NVA). Anything non-value-adding and unnecessary is considered waste (MUDA) and should be eliminated.

During the Discover Phase, consultants conduct a waste walk—a systematic observation of the pathway (on the Gemba)—to identify bottlenecks and NVA activities. These observations are typically documented in the Hospital Resource Form (HRF), which includes a dedicated page for waste identification.

Alternatively, the Waste Walk Poster can be used with the stroke team to visually capture and categorize different types of waste in a collaborative workshop.

This internally developed presentation is designed for Angels consultants and introduces the purpose, structure, and value of in-hospital stroke simulations within the Angels consultancy process. It outlines how simulations are used to safely analyze current practice, test improved stroke pathways, and support patient safety and performance improvement.

The resource provides a high-level, practical overview of how to prepare, run, and debrief a simulation, helping consultants apply a consistent and effective approach when delivering simulation activities in hospitals.

This checklist guides consultants through each phase of the Current State Simulation, highlighting the essential actions, materials and follow-up activities required. Tick boxes for Planning, Briefing, Simulation, Debriefing and Back Office tasks help ensure a consistent approach and that no critical steps are missed.

This checklist gives an overview of all steps of a simulation and which actions and materials need to be considered in each phase. Tick boxes for Planning, Briefing, First Simulation, First Debriefing, Second Simulation and Second Debriefing help to standardise our most important tool and make sure that nothing is forgotten.

A table to be used during a simulation. It lists all required priority actions, and tracks whether they were performed or not, and at which time point after start of the simulation. It also includes fields for Action points and Training for each item, as well as a field to note general objectives and corrective actions discussed during the debriefing.

An excel sheet where the initial time, length and total duration of each action done during the simulation can be tracked. The inserted values are automatically plotted on a bar graph. This provides a comprehensive overview of the timing of each action item, and where life-saving minutes could be saved and the overall pathway improved. 

This legal document gives an overview of the Angels Initiative Simulation Training. The hospital has to sign that they consent to performing a simulation in their hospital and that permission for filming is granted. It is also stated that the recorded video remains property of the hospital unless agreed otherwise.

Legal document for simulated patient. In this consent form, the "dummy patient" agrees to indemnify the hospital, its staff and its associates from any liability that may arise as result of an adverse event that may occur during the course of the dummy runs. They agree to participate as “Simulated Patient” voluntarily and of their own choice. 

Legal document that may be signed by a hospital representative after the simulation. By signing, they consent that all photos, videos and voice recordings can be used by the Angels Initiative for public relations and promotion purposes, such as website or presentations. 

Simulations are performed using the GoPro Hero 5 and the karma Grip to increase capture quality and stability. This document gives precise instructions on how to install the correct GoPro settings and serves as a tutorial on how to properly use the device.

The Simulation Observation or Short Observer Checklist is a practical tool designed for use during in‑hospital simulation sessions. It is intended for participants who are not taking an active role in the scenario.

Observers can use the checklist to focus on specific tasks or team actions—for example, monitoring steps in the pre‑hospital phase or tracking all actions performed by the nursing team. Assigning different tasks to different observers helps ensure that key behaviors and procedures are captured consistently.

The Stroke Team Project Plan is a PowerPoint template used during the Stroke Champions Meeting and the Future State Multidisciplinary Meeting.
It summarizes findings from the Discover Phase, outlines gaps, presents the current state value stream map, sets hospital targets, and provides the basis for identifying and prioritizing improvement projects.

This checklist guides consultants and stroke teams through each phase of the Future State Simulation, highlighting the actions, materials and pathway improvements required to test and validate the agreed future state. Tick boxes for Planning, Briefing, Simulation and Debriefing help ensure a consistent approach and successful implementation of the new pathway.

The Action Plan & Habit Tracker is used during the Implement Phase to organize agreed projects.
For each project, required steps are analyzed using the five Angels platforms. Actions are then arranged chronologically, with deadlines and responsible owners assigned in collaboration with the stroke team.

The Action Plan is provided as a downloadable Excel file, which includes an empty action plan and habit tracker template, examples of different projects aligned with priority actions, and sample habit tracker entries.

The Academia Leaflet is available to support promotion of the eLearning Academy. It outlines the purpose of the academy, key functionalities, and includes a QR code for direct access, along with a course overview.

Digital and print versions—tailored for nurses, EMS and doctors—can be used in hospitals, workshops, and conferences to increase visibility and engagement.

This resource provides a structured method for team reflection called the After Action Review (AAR), widely used in lean and high-performance environments. It is designed to be used during the 30-60-90 Day Formal Reviews within the Feedback Loop of the Angels Lean Consultancy Pathway, supporting continuous improvement through Kaizen principles.

Each visit includes an AAR to review progress, challenges, and next steps. The consultant acts as a facilitator, helping teams build autonomy and a culture of improvement.

A support tool designed to streamline consultancy planning and ensure efficient allocation of field days. 

It consists of an Excel-based calendar that defines available field days and compares them with estimated consulting days based on set regional targets. Public holidays, annual leave, mandatory national and international meetings, internal meetings, and admin days are collated to calculate availability. This helps determine whether plans need adjustment before finalizing and adding all data into the planner.
